Router near miss?

Was doing up the drawer fronts for jewlery boxes today. Using a template, bushing and a 1/4" bit I was going to town when I started seeing metal. I stopped the router, pulled it up and the bit fell out. The collar was still tight so I'm not sure why the bit came out.

My only thought is that you didn't have enough of the bit in the router collet if you weren't taking too heavy of a cut. I had a similar situation happen just this Friday.....

Actually I feel like an idiot. I used a bushing that was too small and I hate using a 1/4" collet and I was too lazy to get the right bushing/bit. Live and learn! Just glad the bit didn't fly out on me.

Similar but all together different... I recently had a bearing come off from a flush trim bit. While it didn't pose the same danger as your mishap, it does go to show that we need to check/tighten our equipment from time to time.
