Hopefully, a mis-print, but in the fine print of the form is the following:

Original sales receipt(s) or sales invoice(s) dated between 08/01/09 and 09/30/09.

Guess some copy writer used an old form :embaresse

:gar-Bi Hadn't noticed that. It's right below the statement purchases must be made between 4/1-6/30/2011. Hopefully someone will point this out to the copywriter.
