Re: Prescription Safety glasses
Walmart or Lenscrafters. Last time I got some (@3 years ago) I gave about $120 at Walmart for mine, metal frame with ANSI lenses and removable side shields - bifocal to boot.

Re: Prescription Safety glasses
This just came up in a thread yesterday, Prescription Saftey Glasses, less than $200, where I recommended ZenniOptical goggles that are around $30 for frames, prescription lenses, and shipping.

Re: Prescription Safety glasses
Any good optometrist should be able to fix you up. After spending the money for a pair several years ago, I since changed to wearing nonprescription safety glasses on top of my presciption glasses. I found that the total weight of the prescription glasses was well above that of my glasses combo.

Re: Prescription Safety glasses
I rarely need my distance correction in the shop so I just alternate between a pair of plain safety glasses and some "reading glass" ones that I bought at the borg when I'm working up close.
Most of the optical shops will try to cram you into polycarbs anyhow (which is what you want anyhow) so you get a dirt cheap set of frames and side shields if you want and it shouldn't cost much.
One thing I forgot the last time is prescription eyewear is all an HSA chargeable item.

Re: Prescription Safety glasses
I got some safety bifocals from Walmart when I bought regular bifocals last summer. Don't remember all the details but the frames were cheap. They had to order them in from the distribution center but it just took an extra four days.
